Nursing Care Plan for Hyperthyroidism - Assessment

 1. Activity or rest
Symptoms: Imsomnia, increased sensitivity, muscle weakness, impaired coordination, severe fatigue
Signs: muscle atrophy

2. Circulation
Symptoms: Palpitations, chest pain (angina)
Signs: Distritmia (vibrilasi atrium), gallop rhythm, murmurs, increased
blood pressure with pressure heavy tone. Tachycardia at rest.
Circulatory collapse, shock (crisis thyrotoxicosis)

3. Elimination
Symptoms: Changes in the pattern of urination (polyuria, nocturia), pain / burning, difficulty urinating (infection), recurrent urinary tract infection, tenderness abdomen, diarrhea, Urine thin, pale, yellow, polyuria (may develop into oliguria or anuria in the event of severe hypovolemia), cloudy urine, odor (Infection), Bowel sounds weak and declining, hyperactive (diarrhea)

4. Integrity / Ego
Symptoms: Stress, dependent on others, financial problems associated with the condition.
Signs: Anxiety sensitive excitatory

5. Food / fluid
Symptoms: Loss of appetite, nausea or vomiting. Not following diet: increased input of glucose or carbohydrates, weight loss of the period of a few days / weeks, thirst, use of diuretics (thiazides)
Symptoms: dry or scaly skin, vomiting, enlarged thyroid (increased pengingkatan needs with blood sugar metabolism), odor or halitosis sweet smell of fruit (acetone breath)

6. Neurosensory
Symptoms: Headache or dizziness, headache, tingling, numbness, weakness in parasetia muscles, impaired vision
Signs: Disorientation, lethargi, stupor or coma (stage), memory impairment (past new) messed up mentally.

7. Pain / Leisure
Symptoms: Abdomen tense or pain (moderate / severe), face grimacing with palpitations, looks to be very careful.

8. Breathing
Symptoms: Feeling a lack of oxygen, cough with / without purulent sputum (Depending on the presence of infection or not)
Signs: shortness of breath, cough with or without sputum purulent (infection),  respiratory frequency increased

9. Security
Symptoms: Dry skin, itching, skin ulcers
Signs : Fever, diaphoresis, broken skin, or ulcerated lesions, decreased general strength / range of motion, muscle paralysis including parastesia or otototot
Respiratory (if potassium levels declined sharply)

10. Sexuality
Symptoms: discharge woman (likely an infection), the problem of impotent in men; orgasm difficulties in women
Signs: Blood Glucose: increased 100-200 mg / dl or more. Acetone plasma: positive prodding. Free fatty acid: lipid levels kolosterol increased.
